What Is Hereditary Spastic Paraplegia (HSP)?

Hereditary Spastic Paraplegia, or HSP, is a group of rare genetic conditions that affect how the brain communicates with the legs.

At its core, HSP is a problem of signal transmission.

The brain sends messages down long nerve pathways to control movement, especially walking. In HSP, those pathways gradually become impaired. The signals still exist, but they do not travel as clearly or efficiently as they should.

What Does That Look Like?

HSP most commonly affects the legs.

 

People may experience:

  • Stiffness or tightness in the legs
  • Difficulty with balance
  • Changes in walking, often described as stiff or “tiptoe” walking
  • Fatigue with movement
  • Increasing need for support such as braces, walkers, or wheelchairs over time

The upper body and thinking are often unaffected in many forms, though some types of HSP can involve additional symptoms.

Why Does It Happen?

HSP is genetic, meaning it is caused by changes in specific genes.

 

These genes are responsible for maintaining the health of long nerve fibers, especially those that travel from the brain down the spinal cord to the legs.

 

When these genes do not function properly:

  • The nerve pathways become less efficient
  • Signals weaken or become disrupted
  • Movement becomes harder over time

There are many different genetic types of HSP, which is why the condition can look different from one person to another.

Is It the Same for Everyone?

No. HSP is not a single disease—it is a family of related conditions.

Some people develop symptoms in early childhood

Others may not notice symptoms until adulthood

Some forms progress slowly, while others are more severe

This variability is one of the biggest challenges for families and clinicians.

Is There a Cure?

Today, there is no cure for HSP.

 

Current care focuses on:

  • Physical therapy to maintain strength and flexibility
  • Supportive devices to improve mobility
  • Medications to help manage muscle stiffness

At the same time, research is advancing. Scientists are working to better understand the underlying biology and to develop targeted treatments, including gene-based approaches.

Why It Matters

HSP may be rare, but the challenges it creates are shared across many rare diseases:

  • Few coordinated resources
